Position Description:
The Care Coordinator will coordinate caregiver referrals with clients and facilitate the provision of exceptional care.
- Coordinates clients’ requests for home care with the availability of independent contractor home care aides to schedule home care services.
- Receives requests for home care services directly from ongoing clients, and through the Intake Department for new clients.
- Verifies specific needs of new clients with the Intake Manager.
- Researches information pertaining to clients’ needs and expectations, gathering information that will assist in effectively matching caregivers with clients.
- Responds to calls from caregivers, asks pertinent questions verifying their accurate availability, and updates database with this information.
- Retrieves On-Call Log, answering service messages and other pertinent information.
- Ensures that all activities and conversations with clients are properly documented by updating database.
- Routinely makes home visits to select clients each month
Qualifications:
- The ideal candidate will have 2-3 years experience working in a doctor’s office or as a customer service or healthcare call center representative.
- Experience with servicing the senior citizen population a plus.
- Bachelor’s degree preferred.
- Proficient with MS Office Suite.
- Data entry skills.
- Detail oriented; able to work independently, meet deadlines and prioritize workload.
- Excellent interpersonal and communication skills, including polite and effective telephone demeanor and customer service abilities.
- Be part of a team and possess a "do whatever it takes attitude."
