Care Coordinator Associate

Family & Nursing Care, a growing, fast-paced home care registry with an excellent reputation for serving the senior community since 1968 seeks a Care Coordinator Associate with a commitment to excellence.

The Care Coordinator Associate will assist the Coordinating Department in providing administrative and customer service support with scheduling nursing referral services to clients.

 

Responsibilities:

  • Contacts and responds to calls from caregivers regarding their availability and skills.
  • Reviews answering service messages daily to identify any matters of concern that arose overnight or on the weekend.
  • Notifies Care Coordinators about any matters that arose with a sense of urgency to facilitate expedient responses.
  • Acts as liaison between Care Coordinators and the On-Call Coordinator, providing instructions, updates, ongoing client engagements, etc.
  • Provides back-up coverage for Care Coordinators when they are out of the office or on vacation.
  • Assists Coordinators with emergency and/or substitute referrals.
  • Ensures all activities or conversations with clients are documented; advises Care Coordinators of any client modifications or changes to their requested services or plans of care
  • Records minutes for Coordinating Department meetings and distributes copies.
  • Types correspondence and manages special projects as needed.
  • Assists with phone coverage for reception as requested.
  • Turns in weekly status reports.

 

Requirements:

  • 2-3 years administrative experience (e.g., in a doctor’s office, as a customer service rep, or healthcare call center rep).
  • Experience with servicing the senior citizen population a plus.
  • Associate degree or higher preferred.
  • Proficient with MS Office Suite.
  • Excellent interpersonal and communication skills.
  • Ability to rectify problems while ensuring the highest level of customer satisfaction.
  • Excellent organizational skills, attention to detail, and ability to multi-task and work well under pressure, despite numerous interruptions and background noise.
  • Able to meet task and report deadlines consistently.
  • Ability to work weekends, as required by a floating schedule.
