RN On-Site Liaison - Senior Living Community

We are looking for a customer service oriented RN who will provide exceptional service and clinical expertise to a senior living community.
Responsibilities
Handle clinical duties in this role and play an active role as the marketing liaison with the client.
Successfully establish, foster and grow the relationship with the client.
Handle the assessing, planning, organizing, administering, documenting, and supervising of client care in accordance with principles and practices of home care nursing.
Develop and implement an individualized care plan for each client including recommendations for appropriate services.
Qualifications
Demonstrated effective customer service skills and caring attitude.
3 - 5 years experience in home care, hospice, and Medicare home health and/or in a private duty home care setting.
3 - 5 years of clinical nursing experience with experience performing patient assessments preferred.
Broad knowledge of the theory and practice of home health care, nursing processes, and nursing practice.
Willingness and desire to work with the elderly; knowledge of the aging process and the physical, psychological, and social needs of the elderly.
Ability to effectively communicate with a variety of people.
Ability to direct client care and services to the elderly.
Excellent time management skills.
Demonstrated ability to perform physical and psychosocial assessment of client and family.
Demonstrated skill in conflict resolution and in building relationships.
Familiarity and understanding of resources for seniors a plus.
Flexibility to adapt to different business expectations, challenges and goals.
Goal and detail oriented; able to work independently, as well as in a tight team setting.
Skilled and effective telephone demeanor and customer service skills to develop a rapport and confidence in the company's services with all callers, clients, and industry professionals.
Demonstrated critical thinking and analytical skills.
Currently licensed to practice nursing in MD.
RN required.
