PERSONAL CARE
  1. Bed Bath/Sponge Bath
  2. Tub/Shower
  3. Shampoo Hair
  4. Brush/Comb Hair
  5. Brushing Teeth
  6. Shave
  7. Apply make up
  8. Nail Care/Clean and File
  9. Skin Care/Non-medicated lotion to dry areas
  10. Assist with getting dressed
  11. Vital Signs
  12. Medication reminder
    (After medicines are set up/dispensed by an RN or family member)

NUTRITION

  1. Prepare meals/snacks
  2. Assist feeding
  3. Feeding
  4. Encourage fluids
  5. Restrict fluids

COMPANIONSHIP

  1. Take for walk
  2. Shopping/Errands
  3. Accompany to appointments
  4. Provide transportation*

    *Caregiver is customarily reimbursed
    at current IRS rate
TOILETING
  1. Assist to bathroom
  2. Assist to bedside commode
  3. Assist urinal/bedpan
  4. Incontinence care
  5. Empty colostomy/catheter bag**

    **Board of Nursing regulations preclude caregivers from assisting with any other elements of colostomy or catheter care.

MOBILITY

  1. Assist with walking (ambulatory)
  2. Turn position in bed (bedridden)
  3. Transfer/Bed/Chair (wheelchair)
  4. Assist with home exercises
  5. Range of motion exercises

SUPPORT SERVICES

  1. Client's laundry
  2. Clean kitchen-Dishes
  3. Take out trash
  4. Maintain bathroom cleanliness
  5. Maintain bedroom/Vacuum/Dust
  6. Oxygen turned on/off only
  7. Maintain living area/Vacuum/Dust
  8. Make bed/change bed linens
