Specialized Home Care for Heart Attack/Disease in Aging Adults
Did you know that more than 25% of heart failure patients1 will return to the hospital within 30 days? The main reasons heart disease and heart failure patients are readmitted to the hospital include poor nutrition, medical oversight, and mobility limitations.
The Caregiver prepares meals for the client in accordance with a meal plan set by a doctor, home health nurse, or nutritionist (e.g. low sodium, low cholesterol, low fat, vegetarian, or vegan), monitors a client’s vital signs, assists with coughing/deep breathing techniques, and encourages routine activity and rest periods as determined by current medical status and/or limitations. Caregivers also take clients to/from follow up doctor visits.
When clients utilize home care, the chances of regaining lost strength, slowing progression of disease, and implementing a healthy new lifestyle are much greater.
