A Safe Transition Home
Coming home after a hospital and/or rehab stay is not easy. People are at risk for dehydration, poor nutrition, weakness, and falls, especially during the first 24-48 hours at home.
The following are a list of programs we provide to help you transition safely:
- Getting Home/Transition Day (starting at just 4 hours of support)
- Pick up at hospital or rehabilitation center
- Escort home (transportation can be provided)
- Medication pick-up at pharmacy
- Grocery shopping
- Unpacking
- Take out trash/throw away spoiled food
- Bathing/toileting/hygiene assistance
- Light housekeeping/laundry/linens change (time permitting)
- Meal preparation (time permitting)
- Medication Reconciliation
- Have a nurse review all your medications to create the most complete and accurate list and avoid any inconsistencies or duplications
- Support To/From Medical Appointments (starting at just 4 hours of support)
- Transport/escort to follow-up doctor appointments
- Additional errands/shopping, as necessary
- Light housekeeping/laundry/ linens change (time permitting)
- Meal preparation (time permitting)
