A Safe Transition Home

Coming home after a hospital and/or rehab stay is not easy. People are at risk for dehydration, poor nutrition, weakness, and falls, especially during the first 24-48 hours at home.

The following are a list of programs we provide to help you transition safely:

  • Getting Home/Transition Day (starting at just 4 hours of support)
    • Pick up at hospital or rehabilitation center
    • Escort home (transportation can be provided)
    • Medication pick-up at pharmacy
    • Grocery shopping
    • Unpacking
    • Take out trash/throw away spoiled food
    • Bathing/toileting/hygiene assistance
    • Light housekeeping/laundry/linens change (time permitting)
    • Meal preparation (time permitting)
  • Medication Reconciliation
    • Have a nurse review all your medications to create the most complete and accurate list and avoid any inconsistencies or duplications
  • Support To/From Medical Appointments (starting at just 4 hours of support)
    • Transport/escort to follow-up doctor appointments
    • Additional errands/shopping, as necessary
    • Light housekeeping/laundry/ linens change (time permitting)
    • Meal preparation (time permitting)
